Try to arrive at least a half hour in advance. We will try to make your visit as convenient as possible. Due to emergencies, delays sometime occur but we make every effort to follow the appointment schedule.

Payment
Payment, including co-payment and anticipated deductibles, is expected at the time of service. If you have any questions regarding our billing procedure please call the office and we will be glad to answer your questions. Accepted forms of payment are cash, check or credit card.

Insurance
Our office participates in Workers' Compensation, Blue Cross/Blue Shield of Rochester, Blue Choice, Medicare, Empire, Preferred Care, Monroe Plan, Valumed and GHI. Medicaid patients are seen on a physician referral basis.

Emergencies
If an emergency that needs medical care arises after office hours, go to the nearest affiliated hospital Emergency Room. The practitioner on call for our practice will be contacted by the E.R. physician. For non-emergency calls which need to be answered after office hours, call the office number and your call will be forwarded to the answering service.

Cancellations
Please let us know if you will be unable to keep your appointment. We will then be able to accommodate others and reschedule your appointment if necessary.

Request for X-rays
If you need to pick up your x-rays or have them mailed, please call the office a few days ahead of time so that these can be prepared for you. When you call please let the office know where and to whom the x-rays are to be sent. There is a $10.00 advanced payment for mailing out x-rays. We cannot process same day requests.

Disability Forms & Release of Medical Records
Disability forms and release of medical records are handled in a timely manner. Please give the office ample time to have these requests completed. There is a $5.00 charge for completing disability forms, paid in advance. We cannot process same day requests.

Any request for release of records must be accompanied by a signed release from the patient. Copies for an attorney must be requested by the attorney along with an authorization for release of records. There is a $.75 per page copying fee for this service (not to exceed 20 papers).
